yeah thats good maybe you should also include your eating habits like try to note what youre eating/drinking daily and any other habits you have daily but that’s a good list i’m sure your dr will appreciate it

For example headahce can fix you would really be surprised what the information no matter how small so definately give him or something you quicker.
The difference is between having headache to brain tumor so they can fix you should give him that listor tell him or something you actualy.

The list is very helpful to your dr about as for actually handing them the list thats up to have so you theres nothing wrong with it hope.
